Male foreskin can be treated through daily cleaning and care, medication, circumcision, foreskin expansion, laser foreskin plastic surgery, etc. Excessive foreskin may be related to factors such as genetic factors, local inflammatory stimulation, abnormal penile development, etc. It usually manifests as symptoms such as inability to evert the foreskin, difficulty urinating, and repeated infections.
1. Daily cleaning and care
Wash the foreskin and glans with warm water every day to keep the area dry and clean to avoid accumulation of dirt. Be gentle when cleaning to avoid damaging the foreskin by pulling it too hard. Choose breathable cotton underwear to reduce friction and irritation. If there is mild inflammation, dilute potassium permanganate solution can be used together with sitz bath.
2. Drug treatment
When combined with foreskinitis or balanitis, you need to use antibacterial drugs as directed by your doctor, such as erythromycin ointment applied topically to fight infection, clotrimazole cream to treat fungal infections, and compound polymyxin B ointment to control mixed infections. Cefaclor dispersible tablets or azithromycin tablets can be taken orally in the acute stage, but the drugs cannot change the anatomical structure of the foreskin.
3. Circumcision
Surgical removal of excess foreskin allows the glans to be exposed naturally, which is suitable for patients with recurrent infections or phimosis. The traditional surgical method uses the scissors method for circumcision. Improved surgical methods such as commercial circumcision can reduce bleeding. Pressure dressing is required for 3-5 days after surgery, and iodophor is used for disinfection to prevent infection and to avoid strenuous exercise that may cause wound dehiscence.
4. Foreskin dilation
For patients with mild foreskin stenosis, dilators can be used to gradually separate foreskin adhesions, and dexamethasone ointment can be used to reduce edema. It needs to be operated by a professional doctor to avoid forcible expansion leading to tearing of the foreskin frenulum. This method is suitable for teenagers who are unwilling to undergo surgery, but the probability of recurrence is higher.
5. Laser circumcision surgery
Carbon dioxide laser is used to precisely cut the foreskin, which has the advantages of less bleeding and quick recovery. Postoperative pain is mild and wound healing time is shortened to 7-10 days. It is necessary to choose a regular medical institution for operation, and conduct regular postoperative reviews to prevent foreskin scar contracture.
Patients with excessive foreskin should avoid forcibly turning up the foreskin on their own to prevent incarceration. During the postoperative recovery period, sexual intercourse is prohibited for 1-2 months, and loose clothing should be worn to reduce friction. The diet should be light and limit the intake of spicy foods. If there are any abnormalities such as pain during urination, swelling of the foreskin and leakage, you need to seek medical treatment immediately. Physiological foreskin hyperplasia in infants and young children does not require premature intervention. If the glans cannot be exposed after puberty, treatment should be considered.
