Testicular pain accompanied by lower abdominal pain may be caused by testicular torsion, epididymitis, varicocele, urinary tract stones, or chronic pelvic pain syndrome. Anti-infective treatment, surgical reduction or analgesia should be taken according to the specific cause.

    1. Testicular torsion:

Sudden severe pain in the testicles that radiates to the groin is a typical manifestation, and it often occurs after strenuous exercise. Rotation of the testicular spermatic cord leads to interruption of blood supply, and failure to reset within 6 hours may cause testicular necrosis. Emergency surgical exploration and fixation are required. Delay in treatment may lead to impairment of reproductive function.

    2. Bacterial epididymitis:

It is caused by ascending infection by pathogens such as E. coli and is accompanied by redness, swelling, heat, pain and fever in the scrotum. Inflammation can spread to the prostate through the vas deferens, causing a feeling of bloating in the lower abdomen. Treatment requires sensitive antibiotics such as levofloxacin, and the acute stage requires bed rest to elevate the scrotum.

    3. Varicocele:

Multiple symptoms on the left side include swelling and pain in the scrotum, which worsens after standing for a long time and radiates to the left lower abdomen. Insufficiency of the spermatic vein valve leads to blood stasis, which may affect testicular spermatogenesis. Severe cases require high ligation of the spermatic vein, while mild cases can be relieved by scrotal support.

    4. Ureteral stones:

When the stone moves, it causes radiating colic, and the pain is transmitted from the waist through the groin to the testicles. May be accompanied by symptoms such as hematuria, nausea and vomiting. Stones smaller than 6 mm can be expedited by drinking more water, while stubborn stones require extracorporeal shock wave lithotripsy.

    5. Chronic pelvic pain:

Pelvic and perineal pain that lasts for more than three months may be caused by prostatitis or pelvic floor muscle spasm. The pain is characterized by vague location, light and sometimes severe, and needs to be improved through comprehensive treatments such as hot compress and biofeedback.

When testicles combined with abdominal pain occur, strenuous exercise should be avoided and loose underwear should be worn to reduce compression. Ice packs can be used temporarily to relieve swelling, but strong painkillers are not allowed to be used to mask symptoms. It is recommended to record the characteristics and duration of pain attacks, and provide detailed medical history when seeking medical treatment. Pay attention to the cleanliness of the perineum every day and avoid riding or sitting for long periods of time. Increase your vitamin C intake in your diet to prevent urinary tract infections, and limit high-oxalate foods to prevent stone formation. If the pain lasts for more than 6 hours or is accompanied by high fever or hematuria, emergency treatment is required immediately.
