The chronic disease record card we often refer to is officially called the Chronic Disease Outpatient Benefit Qualification Certificate. Most of the cards can be obtained directly at the medical insurance service points in the insured areas. Now, designated treatment hospitals in many areas have also opened direct channels, so there is no need to go to the medical insurance bureau.

Not all places have such a convenient policy. I have seen many netizens complain that my city has not yet opened the permission to apply for other places. I have been working and settling in other places all year round. To apply for a chronic disease card, I have to go back to my hometown. Not to mention the back and forth, some places also require a bunch of residence certificates and work certificates to be issued in advance, which is quite time-consuming.

If you don’t want to go offline, you can first check whether there are online channels in the local area. Now many provinces and cities have launched online registration portals on the official medical insurance mini-program or government service platform. For example, friends in Zhejiang can directly search for “chronic special disease registration” on the “Zhejiang Office” and upload the corresponding examination report and confirmed medical record. The review results will usually be available in 1-2 working days. You can directly use the electronic chronic disease card to go to the hospital to prescribe medicine without even leaving the door.

By the way, before applying, it’s best to call 12393 to find out the specific local requirements, such as whether you need to bring inspection reports from the past six months, and whether there is a designated hospital. Don’t go there with medical records from several years ago that do not meet the requirements and make a wasted trip.
