A testicle that is high on one side and low on the other may be caused by diseases such as varicocele, cryptorchidism, testicular hydrocele, epididymitis or testicular tumors. It is recommended to seek medical examination promptly and carry out targeted treatment after clarifying the specific cause.
1. Varicocele
Varicocele is caused by abnormal expansion of the venous plexus due to obstruction of blood return in the spermatic veins, which can cause scrotal ptosis and low testicular position on the affected side. The disease may be related to venous valve insufficiency, prolonged standing and sitting, and other factors, and is often accompanied by a feeling of bloating in the scrotum. Treatment includes bed rest, scrotal band immobilization, and laparoscopic high spermatic vein ligation if necessary.
2. Cryptorchidism
Cryptorchidism means that the testicles fail to descend into the scrotum and may remain in the groin or abdominal cavity. Unilateral cryptorchidism after puberty will lead to significant asymmetry in the position of the bilateral testicles, and the undescended testicles may atrophy. The disease is related to congenital developmental abnormalities and requires surgery to fix the testicles to the scrotum. The optimal time for intervention is between 6 and 12 months of age.
3. Testicular hydrocele
Testicular hydrocele is excessive accumulation of fluid in the hydrocele, which increases the volume of the scrotum on the affected side and leads to abnormal testicular position. Primary effusion may be related to the imbalance of secretion and absorption of the vaginal membrane, and secondary effusion is often caused by inflammation or trauma. A small amount of asymptomatic effusion does not require treatment, but a large amount of effusion requires intrathecal inversion surgery.
4. Epididymitis
Acute and chronic epididymitis can cause redness, swelling, heat and pain in the scrotum on the affected side, and the inflammatory reaction may cause the testicles to move upward or become abnormally fixed. The disease is mostly caused by bacterial retrograde infection. It can be treated with antibiotics such as levofloxacin tablets and cefixime dispersible tablets as directed by the doctor. In the acute stage, bed rest and scrotum elevation are required.
5. Testicular tumors
The growth of testicular tumors will increase the size and hardness of the affected testicle, destroying the normal anatomical position. Tumors such as embryonal carcinoma and seminoma may be accompanied by a feeling of heaviness in the testicles. The diagnosis requires ultrasound examination and tumor marker detection. The main treatment is radical orchiectomy, followed by radiotherapy and chemotherapy according to the pathological type.
In daily life, you should avoid riding a bicycle or sitting for a long time, and choose loose and breathable underwear to reduce scrotal compression. Testicular self-examination can be performed once a month. Use a standing position to gently palpate the testicles with both hands to compare the size and position differences between the two sides. If you find that your testicles suddenly increase in size, harden in texture, or have persistent pain, you should see a urology department immediately. Maintaining moderate exercise can help improve pelvic blood circulation, but it is necessary to avoid scrotal trauma caused by strenuous oppositional exercise.
